Definition of Disabled Child

Any child of  any age and mentally retarded or physically and permanently incapacitated and is incapable of supporting himself disabled 

Procedure

1. Heir must submit an application indicating the type of disability of the child.

2. The relevant State Department of Social Welfare (JKM) would be required to investigate.

3. Heir must bring the child to a Government Medical Specialist to obtain a Report on Medical Specialist  Examination.

4. Documents required:-

• 1 copy of Birth Certificate of the child and identification card of child above 18 years old.
   
• A full length photograph of the child showing physical attributes.
   
• 1 copy of identification card the heir or guardian (if child is not cared for by the natural mother/father or the natural mother/father has passed away).
   
• Complete Guardian Declaration Letter (JPA/PT9) available from Pension Division, PSD.  This form is only required of the guardian who  is not the  natural mother/father.
