PENSION SCHEME :: EXISTENCE OF MORE ELIGIBLE DEPENDANTS

If the pensioner or pension recipient dies, the next of kin must immediately and officially inform the Pension Division, PSD.  Application for Derivative Pension must be made with the submission of the following documents:
 

  • 1 copy of Derivative Pension Application for in Service Death (Public/ Statutory Body)  - JPA.BP.SPT.B01a Form

  • 1 copy of PEN 14 form
    Both forms are available free of charge at any Government department or Pension Division, PSD.

  • 1 copy of the Death Certificate of the deceased.

  • 1 copy of Marriage Certificate or fill the form Widow/Widower Declaration form (if no marriage certificate).This form is available at Pension Division; 

  • 1 copy of Divorce Certificate/Reconciliation Certificate whichever the case 

  • 1 copy of identification card of widow /widower;

  • 1 copy of birth certificates of only children who are eligible. For child between the age submit a copy of identification card 

  • 1 copy of Adoption Registration Certificate, if relevant).

  • Pensioner Card.

  • Warrants that have not been cashed, if any. 

  • Pension Payment via the Bank (JPA.BP.SPT.B06  Form) available at Pension Division, PSD.
     

All documents submitted must be endorsed by any of the following official: 

* Kadi, Penghulu, Penggawa, Chief of Police Station, Headmaster, District Officer, Government Officer in the Management and Professional Group or any Government Head of Department. The full name, post and department chop of the officer must be clearly stated.
