PROCEDURE FOR MANAGING DOCUMENTS FOR APPLICATION OF PENSION BENEFITS

  • The use of POWER has brought about several changes in the management of  pension documents used in the application of pension benefits. Under POWER several new documents have been introduced, which enable the documents being sent via electronic media or/and manually.  The forms required may be downloaded from the portal/web and printed. 

  • To facilitate agencies in preparing of complete documents prior to the submission of the application for retirement and pension benefits to this Department, listed below is the list of documents according to the types of retirement.  This list should be used as a checklist by all agencies in managing this matter:

 

Compulsory retirement upon reaching the age of 55 or 56 years [Section10(1)]

Application documents required are:

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Release / Separation Notice of Government copntribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of marriage certificate

  • A copy of birth certificate of children below the age of 21 years.

  • A copy of identification card

  • EPF 1036 Statement ( KWSP 1036)

 

Compulsory Retirement due to incapability of discharging the duties of his office, by reason of  infirmity of mind or body likely to be permanent  [Section 10(5)(a)]

Application documents required are:

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Release / Separation Notice of Government copntribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of identification card

  • EPF 1036 Statement ( KWSP 1036)

  • Comments from Head of Department if officer disagrees to be pensioned off.

 

Compulsory Retirement due to the abolition of the office held by him [Section 10(5)(b)]

Application documents required are:

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Release / Separation Notice of Government contribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of marriage certificate

  • A copy of birth certificate of children below the age of 21 years.

  • A copy of identification card

  • EPF 1036 Statement ( KWSP 1036)

  • Letter for the confirmation of free from any Disciplinary / Criminal Action

  • Letter for officer's option

  • A copy of approval letter from Service Division, PSD on the abolition of post

 

Compulsory Retirement for the purpose of re-organisaation of the department to which the officer belongs [Section 10(5)(c)] on the ground that the officer's employment has been terminated in the public interest [Section 10(5)(d)]

Application documents required are:

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Release / Separation Notice of Government contribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of marriage certificate

  • A copy of birth certificate of children below the age of 21 years.

  • A copy of identification card

  • EPF 1036 Statement ( KWSP 1036)

  • Letter for the confirmation of free from any Disciplinary / Criminal Action

  • Letter for officer's option

  • A copy of approval letter from Service Division, PSD on the abolition of post

 

Compulsory Retirement on the ground that the officer has voluntarily acquired citizenship of another nation [Section 10(5)(e)]

Application documents required are:

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Dependants' Information Form - JPA.BP.SPPP.B03

  • Release / Separation Notice of Government contribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of marriage certificate

  • A copy of birth certificate of children below the age of 21 years.

  • A copy of identification card

  • A copy of passport

  • EPF 1036 Statement ( KWSP 1036)

  • An extract of the minutes of meeting on the decision of the Disciplinary Board  to terminate the officer's services due to acquiring foreign citizenship (for Statutory Bodies and Local Authorities and State services)

  • Letter of department recommendation to retire the officer due to him acquiring foreign citizenship (for Statutory Bodies and Local Authorities services) 

  • Letter of approval to retire the officer due to him acquiring foreign citizenship by state authorities (for state public service only) 

  • Memorandum on the approval of retirement from the Service Division, PSD ( for federal public service only)

  • Other documents relating to officer's disciplinary case

 

Compulsory Retirement on the ground that the officer had for the purpose of appointment to the public service provided falsse infformation [Section 10(5)(f)]

Application documents required are:

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Dependants' Information Form - JPA.BP.SPPP.B03

  • Release / Separation Notice of Government contribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of marriage certificate

  • A copy of birth certificate of children below the age of 21 years.

  • A copy of identification card

  • EPF 1036 Statement ( KWSP 1036)

  • An extract of the minutes of meeting on the decision of the Disciplinary Board  to terminate the officer's services on the ground that the officer had provided false information (for Statutory Bodies and Local Authorities and State services)

  • Letter of department recommendation to retire the officer on the ground that the he had provided false information  (for Statutory Bodies and Local Authorities services) 

  • Letter of approval to retire the officer due to him  providing false information  (for state public service only) 

  • Memorandum on the reduction of retirement benefits from the Service Division, PSD ( for federal public service only)

  • Other documents relating to officer's disciplinary case

Retirement at the instance of the Government or Minister Under Section 11 Act 227/239 - on the ground of national interest [Section 11(a)(i)]

Application documents required are:

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Release / Separation Notice of Government contribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of identification card

  • EPF 1036 Statement ( KWSP 1036)

  • Letter of Agreement

  • Decree of Appointment from the Government

 

Retirement at the instance of the Government or Minister Under Section 11 Act 227/239 - in the interest of the public service or Statutory Bodies or Local Authorities 

Application documents required are:

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Dependants' Information Form - JPA.BP.SPPP.B03

  • Release / Separation Notice of Government contribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of identification card

  • EPF 1036 Statement ( KWSP 1036)

  • Letter of officer's agreement to be retired;

  • Officer's performance report from the Head of Department;

  • Medical report; and

  • Approval letter from the Disciplinary Board in Service Division, PSD regarding the termination of the officer's service under this regulation.

 

Optonal Retiremnt Under Sectionn 12 Akta 227/239

Retirement application documents required are: 

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Release / Separation Notice of Government contribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of birth certificate of children below the age of 21 years.

  • A copy of identification card

  • EPF 1036 Statement ( KWSP 1036)

  • Letter for the confirmation of free from any Disciplinary / Legal/ ACA Action (for A & B categories)

  • Optional retirement approval letter (for sate public service)

 

Retirement Due to Privatisation of a Government Agency Under Section 10(5)(b), 12 and 12A/13 Act 227/239 – Refer to Service Circular No. 6 of Year 2000.

Retirement application documents required are: 

  • Record of  Service / Leave.

  • Pensioner Information Form – JPA.BP.SPPP.B01

  • Debtors /Non Debtors Declaration Certificate - JPA.BP.UMUM.B01

  • Release / Separation Notice of Government contribution in the Employees Provident Fund for computation of pension benefits  - JPA.BP.SPPP.B04

  • A copy of marriage certificate

  • A copy of birth certificate of children below the age of 21 years.

  • A copy of identification card

  • Letter for the confirmation of free from any Disciplinary / Legal Action 

  • EPF 1036 Statement ( KWSP 1036)

  • Option Paper

  • Supporting document if Applying for early retirement benefits [Section 12A(4) Act 227/Section 13(4) Act 239] due to ill health: Report from Medical Board and  Service Termination Letter from Employer

  • Reduction of Employees: Letter of Agreement and Approval from the Government

  • An extract of the minutes of meeting on the decision of the Disciplinary Board  to terminate the officer's services due to acquiring foreign citizenship (for Statutory Bodies and Local Authorities and State services)

  • Letter of department recommendation to retire the officer due to him acquiring foreign citizenship (for Statutory Bodies and Local Authorities services) 

  • Letter of approval to retire the officer due to him acquiring foreign citizenship by state authorities (for state public service only) 

  • Memorandum on the approval of retirement from the Service Division, PSD ( for federal public service only)

  • Other documents relating to officer's disciplinary case

 

Dies in Service

Retirement application documents required for married officers with dependants are:

  • Derivative Pension Benefits Application for Death in Service (Public/ Statutory Body) – JPA.BP.SPT.B01a

  • Record of  Service from the last department served   and  Leave Record 

  • Debtors /Non Debtors Declaration Certificate– JPA.BP.UMUM.B01

  • A copy of Death Certificate

  • A copy of marriage certificate

  • A copy of widow's/widower's identification card 

  • A copy of birth certificate of child/children below the age of 21 years and unmarried.

Retirement application documents required for unmarried officers are:

  • Derivative Pension Benefits Application for Death in Service (Public/ Statutory Body) – JPA.BP.SPT.B01a

  • Record of  Service from the last department served   and  Leave Record 

  • A copy of mother's/father's/heir's identification card

  • A copy of Death Certificate

 

Disability Pension

Retirement application documents required are: 

  • Police Report on the accident, whether is due to the negligence of the officer or otherwise 

  • Medical Report on the degree of  injury.

 

Dependant's Pension

Retirement application documents required are: 

  • Information record on the deceased whose  derivative pension benefits have been approved

  • A copy of  the police report by the heir

  • Police Report Form for fatal accident

  • Confirmation Report from the head of department (if any) 

  • Inquest (if required)
